Doctors' Association UK is the controller for this survey. Version 2026-08-23-draft-5.
What we collect
Every response includes the organisation, nation, your role category and your answers. Unnamed mode does not collect your name, email address or job title.
Named mode collects your name, work email and role title, and whether we may contact you or quote you. A management code is issued only in named mode so you can later request a correction or withdraw the response.
At final review, you may optionally upload up to three evidence files. Files are held in private storage for authorised DAUK review. Do not include patient information or personal information about another person.
Why we collect it
We use the data to describe how physician associate roles are governed after the independent review. We rely on legitimate interests under UK GDPR Article 6(1)(f). Separate permission covers optional follow-up, quotation and attribution.
How long we keep it
Raw responses, optional evidence files and named contact details are kept for 12 months after fieldwork closes, unless a correction, legal or safeguarding hold applies. Contact details are deleted when follow-up and right of reply are complete. A record of findings with contact details removed, the methodology, source list, organisation list and published aggregate data are kept for seven years.
Vercel, Supabase and network providers process connection data to deliver and protect the service. To prevent automated abuse, a keyed hash of the connection address is retained briefly for rate limiting. It is not joined to survey answers.
Your rights
You can ask for a copy of personal data we hold about you, ask us to correct it, or object to further use. Named respondents can request a correction or withdraw a response with the receipt number and management code on the manage a response page. DAUK does not collect contact details for unnamed responses and cannot authenticate the respondent through this form, so they cannot be withdrawn here.
